your child’s Kindergarten year. Please bring this information with you to your appointment:
✓ proof of Ontario residency, such as a rental or sales agreement, or current utility, phone
or tax bill
✓ proof of Status in Canada, such as a Canadian birth certificate or Canadian passport;
✓ immunization records
✓ your child’s Health Card number
✓ child care information (if applicable)
✓ emergency and other contact information
